AGREEMENT BETWEEN PURCHASER AND USER OF GIFT CARDS FROM THE SITE AND CASCADE WELLNESS LLC

Gift cards are available for non-insurance services and in-office products offered through Cascade Wellness LLC. This includes acupuncture and Chinese medicine for cash-paying patients, nutrition services, VO2 Max, RMR services, and products that Cascade Wellness may have available for purchase in office. We are legally not permitted to accept gift cards for insurance co-payments, coinsurance, or deductible.

By purchasing a gift card you agree that the card is non-refundable, cannot be exchanged for cash, and is treated as cash (not replaced if lost or stolen). Other terms and conditions include the following, by purchasing a gift card, you agree to these terms and conditions.

• Redemption: only valid for cash services offered by Cascade Wellness LLC, not valid for services where insurance is accepted.

• Expiration: valid for 2 years from the date of purchase.

• Returns: any product or item that is available in our office and purchased with a gift card cannot be returned (e.g., Evil Bone Water or DermaRollers).

• Fraud: we reserve the right to cancel or reject gift cards if we suspect fraud, misuse, or unauthorized distribution.

Gift cards purchased through Home Skincare and Wellness intended for use for products and/or services offered by Cascade Wellness are subject to the above Terms and Conditions. An extra fee of no more than $5 on top of the products and services offered will apply due to using an outside gift card for services offered by Cascade Wellness LLC.

Cascade Wellness LLC reserve the right to update these terms and conditions at any time. Continued use means you accept the changes.

AGREEMENT BETWEEN PURCHASER AND USER OF CONTINUING EDUCATION PROGRAMS FROM THE SITE AND CASCADE WELLNESS LLC

Continuing education programs are offered by Cascade Wellness LLC, for purchase on www.cascadewellnessmd.com, for practitioners of Chinese Medicine to be used to fulfill continuing education credits as required by individual states and/or NCBAHM. By purchasing and/or using a continuing education program from Cascade Wellness LLC, you agree to this binding agreement and the following terms and conditions.

• User rules: illegal activities, abuse, or unauthorized use of the website and programs within is strictly prohibited.

• Intellectual property: All content (text, graphics, logos) is the property of the Site and protected by copyright laws. You may not use our content without permission.

• Termination: we reserve the right to terminate accounts that violate these terms without prior written notification and/or reimbursement.

• Limitation of liability: Cascade Wellness LLC is not responsible for any damages resulting from your use of this site and it's programs. The Site and related continuing education program content is provided "as is" without warranties.

• Refund/payment policy: once purchased an online continuing education program cannot be refunded. Payment for a continuing education program is due upon purchase.
